Issue:
The VA Pharmacy Service staff reported that Veteran A received medications and an instruction sheet on Veteran B. Both patients receive the same blood pressure medication with the same directions. Veteran A found the error and held the prescription along with the paperwork. None of the medication was consumed. The medication and the paperwork were given to the Home Health Nurse during a Home Health visit on 08/10/12. The Home Health Nurse returned the medication and the paper work to Pharmacy Service. Veteran B was mailed a medication replacement. The mismailed package included the medication and instruction sheet with Veteran B's full name, address and information on the medication. Update: 08/15/12:Veteran B will be sent a notification letter.
Outcome:
Pharmacist responsible for mishandling/mis-mailing error was counselled by Pharmacy Svc. supervisor (per e-mail on 8/14/12).
