Issue:

Veteran A received Veteran B's narcotic contract and other papers. Full name was involved. The Privacy Officer (PO) is unable to determine the other types of information involved and will provide an update when Veteran A is contacted. The information was outside of VA control for more than 72 hours. Update: 02/08/12:The narcotic contract contained Veteran B's name, address, full SSN , date of birth. There was also a progress note that contained 7332 information. The documents were recovered and shredded. Veteran B will receive a letter offering credit protection services.

Outcome:

Re-educated facility staff that might have been responsible for this. Attempted to reduce the sensitivity of the documents that are being sent. (last 4 of SSN rather than full SSN).
