The History and Physical Progress note of an inpatient Veteran was found on top of an isolation cart on Medicine ward area. Unable to determine responsible party of failure to safeguard/disposal of Veteran PHI. Update: 09/28/11:Due to full SSN and DOB being exposed, Veteran A will be sent a letter offering credit protection services.
