Issue:
A Veteran's name and medical information was inadvertently included in Orlando VAMC bi-monthly Veterans Health Education newsletter. The information identified on the newsletter (Vet Gazette) was the Veteran's full name, medication and UPS shipping tracking number for the medication mailed to the home of the Veteran.
Outcome:
11/17/14: The Incident Resolution Service Team has determined that the Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
