Issue:

Veteran A telephoned the Medical Records Department to report that he received information on Veteran B along with his own medical information. Veteran A reported to the employee on the telephone that he has mailed Veteran B's information back to the Medical Records section. The Privacy Officer (PO) is completing a fact finding and has not yet determined what information has been disclosed inappropriately. Update: 09/17/12:The Privacy Officer is waiting on the information to be returned. When it is, there will be a determination if any notification needs to be sent to Veteran B.09/24/12:Confidential medical diagnosis along with other sensitive information and identifiers, including the SSN were mailed to the wrong Veteran. The Veteran affected will be offered credit protection services.

Outcome:

Responsible employee counseled by PO and required to re-complete Privacy and Information Security Training. This has been reported to the Chief of Labor Relations and supervisor for any further disciplinary action. Credit monitoring letter completed.
