Issue:
Veteran A contacted his healthcare provider to follow up on an upcoming appointment. The provider could not confirm and during the conversation it was determined that Veteran A received a copy of Veteran B's appointment list. Veteran A acknowledged the appointment list was received in-person from a medical support clerk while on station.
Outcome:
01/23/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
