Issue:

A Veteran submitted a claim for reimbursement of travel expenses and receipts to the clinic. The Veteran handed an envelope containing the claim and receipts to a VA employee who acknowledged it was received. The employee reported the envelope was turned over to the clinic Travel Clerk. The clinic Travel Clerk reported this type claim would have been sent to the Medical Center for processing. After a thorough search, neither the clinic Travel Clerk nor the medical center staff can locate the form and receipts. Privacy will further investigate.

Outcome:

12/18/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
