Issue:
Based on interview, and hospital document review, the hospital failed to maintain the confidentiality of protected health information for Patient A. This resulted in the patient's protected health information being known to unauthorized person without the patient or the patient's representative permission. Findings:On 12/13/12, the unit coordinator learned one of the hospital's employee reviewed Patient A's medical record. The hospital employee was a family member of Patient A; however, the employee did not have permission to review the patient's medical record from the patient's immediate representative. During an interview and documentation review on 4/29/14 at 1615 hours, the Manager of Regulatory Compliance confirmed the above event.
