Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one person (Patient A), when a staff member accidently sent Patient A's medical information to the wrong recipient.Findings:An interview was conducted on September 25, 2012 at 9:45 AM, with the facility's Privacy Officer (PO). The PO stated that on September 12, 2012, she became aware of a breach incident. The PO stated that on September 12, 2012, a staff member accidently sent a copy of Patient A's medical information intended for Patient A's physician in Mammoth to another physician with the same name. The PO stated that the breach was identified when the receiving physician with the same name called the facility and stated that this was not his patient.A review on September 25, 2012 of the personal health information (PHI) disclosed in error included:Patient nameMedical record numberUltra sound reportThe PO further stated that the receiving physician shredded the medical information and Patient A was notified of the breach.
