Issue:

Beckley's VAMC's travel clerk cannot locate six completed employee travel packets. Each individual packet contains a completed SF 182 as well as either Attachment A or C to the SF 182, the employees travel itinerary and a completed employee expense report with receipts. The employee's full SSNs are on the SF 182. Update: 01/04/11:Due to full SSN being exposed, six (6) employees will receive a letter offering credit protection servicesNOTE: There were a total of 17 Mis-Handling incidents this reporting period. Because of repetition, the other 16 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate

Outcome:

The employee responsible has received training. Additionally, this incident has certainly heightened her awareness of how easy it is to accidentally shred documents that do not require shredding. Some of the routine office procedures have also been changed, in that such documents have been moved into a different cabinet so that it is not as easy as it was to mix up "to be shredded" paperwork with current paperwork that is still being worked on. This unit will be moving from the hospital facility to an off-site leased space within the next couple of weeks. The new office is bigger and this will provide the employee more room to keep her work organized. Her present office is very small.
