Issue:

Pathology staff reported to VA Police that a camera was missing from the morgue. The camera in question contained several photos of an autopsy being conducted on a deceased Veteran. Staff noticed this camera missing on 05/05/15 but didn't report to Police Services until 05/21/15.

Outcome:

05/22/15: The Incident Resolution Service Team has determined that the Veteran's Next of Kin will be sent a notification letter, as his full name and SSN were stored on an image on the camera.
