Veteran A received a copy of Veteran B's and Veteran C's diagnostic follow up letters by mail in error. The letters were attached to Veteran A's. The letters are stuffed in envelopes and mailed out by Volunteers within the Women's Clinic.
Outcome:
12/30/13: Veteran B and C will be sent a notification letter.
