Issue:

The Privacy Officer received a message to call a Veteran. Upon contact, Veteran A explained that he requested a copy of the medical record on a CD. Veteran A received the CD of Veteran B which contained medical documents, including Veteran B's SSN. Veteran A will return the CD when he comes back to the hospital on Tuesday, August 25, 2011 as he is unable to return immediately. Veteran A has expressed concern that the CD he requested is now in someone else's hands, and feels he should be given credit monitoring.PO is requesting that both Veteran A and Veteran B receive credit monitoring as the location of Veteran A's CD is unknown at this time and Veteran A is in possession of Veteran B's information. Update: 08/18/11:Veteran B will be sent a letter offering credit protection services.

Outcome:

Supervisor is conducting additional training for employee who mis-mailed the documents. Additionally, employee is going to receive written counseling due to this breach. Redacted credit monitoring letter attached.
