Issue:
A patient gave a hand written paper to a counselor today that contained the full name, full SSN and full date of birth of 5 other patients that are in this program. He told them that he had written down in information and had looked at the wristbands to get the information. He read it off the wristbands and copied it down. When he gave the paper to the counselor he said "How do you know I don't have 10 copies of this?" He has since left the hospital. Update: 03/24/11:Five (5) Veterans will be sent letters offering credit protection services.
Outcome:
Credit monitoring letters sent out. Patient removed from program.
