Issue:

Based on document review and staff interview the facility failed to prevent unauthorized disclosure of patients health information (PHI) for 6 of 6 patients (1-6). Findings:1. On 6/11/13 the facility reported that on 6/6/13 Patient 1's (MRN: XXXXXXX) discharge instructions containing PHI were given to another patient.2. ON 8/14/13 the facility reported to the Department that an employee dialed a wrong number and left a message containing PHI for Patient 2 (MRN: XXXXXXX) on the wrong voicemail.3. On 12/10/13 the facility reported to the Department that on 12/6/13 discharge instructions for Patient 3 (MRN: 4418719) were inadvertently handed to another patient. 4. 3/13/14 the facility reported to the Department that on 3/10/14 Patient 4's (2014-02302) prescription containing PHI was given to another patient. 5. On 6/4/14 the facility reported to the Department that on 5/29/14 a nurse left her workstation (a computer) unsecured and Patient 5's (MRN: XXXXXXX) mother used the workstation to view the patient's PHI. 6. On 9/19/14 the facility reported to the Department that on 8/12/14 (detected on 9/17/14) a disc containing Patient 6's (case number: 2014-09002) PHI was released to another patient. During an interview on 5/7/15 the facility's Accreditation, Regulation and Licensing liaison confirmed that the above breaches occurred as reported to the Department.
