Issue:

A VA patient left the Dermatology Clinic with his medication profile. The patient also picked up the printed appointment list that was on the doctor's desk. The doctor realized the appointment list was missing, The nurse called the patient who was he driving home. He stated he will look at his paper work when he arrived home. He called back to say that he has the list and the nurse told him he could use his personal shredder to destroy the list. The appointment list had eight patients' name, full SSN and home phone number. Update: 07/21/11:The eight patients will receive a letter offering credit protection services.

Outcome:

Appointment lists are kept in a plastic sleeve to prevent veterans from accidentally picking it up.
