A Release of Information (ROI) staff member mailed Veteran A's responsive records to Veteran B. Veteran A's information was at the bottom of Veteran B's records. Veteran B spoke with ROI and stated that he will mail Veteran A's records back to the facility.
Outcome:
05/19/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services due to full SSN being disclosed.
