Issue:
Veteran A presented to local community-based outpatient clinic (CBOC) and stated that approx. one month ago while at the main clinic, he was given Veteran B's appointment list by mistake, which he didn't discover until he went for what he thought was his next appointment, on 1/30/13. He was subsequently told he didn't have an appointment, but didn't connect that fact until he examined the name at the top of the listing and discovered it belonged to Veteran B. His next appointment was today, 1/31/13, whereupon he informed the clerk of the error, who in turn relayed to the Privacy Officer. The list contained Veteran B's full name, last four SSN, and nine future appointments, with clinic names (i.e. cardio, GI procedure, podiatry, etc.). We are now attempting to trace where this error occurred. The appointment list was recovered. Update: 01/31/13:Veteran B will be sent a HIPAA notification letter.
Outcome:
Event will be briefed at next staff meeting to enhance clerks overall awareness.
