Issue:

On 07/31/014, Veteran A picked up a large packet of his medical records from the Release of Information (ROI) Office. When Veteran A got home, he took a while to start reviewing his medical records. Upon reviewing those medical records, he then noticed within his records were 92 pages of medical records pertaining to Veteran B. Upon researching further, it was noted that 92 pages of incorrect medical records of Veteran B, were scanned inadvertently into the record of Veteran A. During the processing of his ROI request, the incorrect records of Veteran B were printed and provided to Veteran A. The PO has reviewed those medical records, which were records from an outside facility and included Veteran Bs full name, full SSN, date of birth, address, telephone number and all medical information within those records. A self-address tracked overnight envelope is being sent to Veteran A for the return of the records of Veteran B.

Outcome:

08/08/14: The Incident Resolution Service Team determined that Veteran B will receive a letter offering credit protection services.
