Issue:

Veteran A received medications at the Frank M. Tejeda VA Outpatient Clinic (FTOPC) window (Ibuprofen) belonging to Veteran B (same last name, different first name). The error was discovered very shortly thereafter and pharmacist telephoned Veteran A to tell him that he had wrong prescription. Veteran A had not opened the prescription bottle and recognized it was not intended for him. He returned the wrong prescription in approximately 30 minutes. He was given his correct prescription for Lisinopril. Veteran A did not ingest any of the wrong prescription and therefore no harm to patient occurred. FTOPC pharmacist is writing a report of contact and will forward to Privacy Officer (PO).

Outcome:

06/18/13: Veteran B will be sent a notification letter.
