Issue:
While processing a Release of Information (ROI) request, the incorrect patient was chosen and information was sent to the attorney requesting the information. The attorney's office contacted us upon receipt of the information and stated that they shredded the information and requested to correct patient's records. The information that was compromised included the Veteran's name, address, full SSN and diagnosis. Update: 01/06/12:No data breach has occurred. The document was seen only by VA trusted agents.Additional information was received to indicate that the attorney was a public attorney who has not been vetted by VA and found to be authorized to stand before the VA. The Veteran will receive a letter offering credit protection services.
