Issue:
Based on interview and record review, the facility failed to prevent unauthorized access and disclosure of four patients' (Patient 19, 20, 21 and 22) protected, confidential health information, when the patients' information was available to person/s who were not involved in the patients' care and not authorized to receive the information. This failure allowed for potential unlawful or unauthorized use of the information and violation of the patients' right to privacy. Findings: During an interview on 8/26/14 at 10 a.m., the Compliance Officer confirmed that the patients' information had been included in property which was in an employee's purse and was stolen from the employee's vehicle. The information was recovered, but it is not known if it was read. The Compliance Officer confirmed the information which was included.The interview and a review of the letter submitted by the facility revealed that the information included the Patients' names, date of birth for Patients 19 and 20 and medical record number for Patients 21 and 22.The incident resulted when information was in a place which vandals were able to access.
