Issue:
Veteran A contacted the Privacy Officer, Nashville Campus, regarding the following concern: While viewing her records in MyHealteVet, Veteran A found a document belonging to Veteran B; specifically, a Cardiac Catheterization Lab Hemodynamics note dated August 22, 2014. Veteran A requested the erroneous information be removed from her record and assurance her information was not inadvertently placed in Veteran Bs records. Upon inquiry, the Privacy Officer (PO) found Veteran A and Veteran B had a cardiac catheterization completed on the same day. The technician inadvertently uploaded Veteran Bs report into Veteran A records. The erroneous note has been removed from Veteran As record. The PO reviewed Veteran Bs records and confirmed they do not contain Veteran As report. Veteran Bs personal identifiable information includes: full name, full social security number, date of birth, address, vital signs, ethnicity, lab results, and procedure note
Outcome:
09/01/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
