Issue:
Patient A was seen in the Emergency Department (ED). While in the ED, the patient was given Patient B's medical wrist band by a VA employee. Patient A was provided medical care but left the ED prior to full treatment. When Patient A arrived home, he realized the medical wrist band belong to Patient B. Patient A returned to ED and VA employee removed Patient B's wrist band from Patient A. The wrist band contained Patient B's name, date of birth and full SSN.
Outcome:
10/03/13: The wrist band contained Patient B's full name and full SSN which Patient I left the facility with. Patient B will be sent a letter offering credit protection services.
