Issue:

I received email from VA employee stating she was concerned with a Data Overwrite Incident.As we discussed on the phone, I was informed of a demographic data overwrite of one of our patient files this afternoon. From what I understand, veteran A used the www.usgov.com website to register online using the 1010EZ. This website sends to our system the veteran's information to be processed by the EZ 1010EZ processing option in VISTA (Employee Ain Registration processes these). This processing appears to have overwritten another veterans Veteran B demographic information. Both Veteran A and Veteran B have the same first and last name. Based on a inquire with audit on the Fileman PATIENT file, this happened on F02/04/11. Update: 02/14/11:This error caused Veteran B's medication to be sent to Veteran A. Veteran B will receive a letter of notification.

Outcome:

I called the VA National Service Desk, and they fixed the data overwrite problem. Brought this incident up to morning report and educated them to have their employees be more cautious.
