Issue:

A Field Service Officer (FSO) from the Disabled American Veterans brought a CD containing Veteran A's medical record and Veteran B's medical record. Veteran A, who brought the CD to the FSO did not open the package the CD came in. The FSO opened the package and loaded the CD into her computer. When she opened the directory there were both Veteran's medical records on the CD ROM. The FSO did not open Veteran B's medical record and brought the CD to the Privacy Officer. Update: 03/16/12:Veteran B will be sent a letter offering credit protection services, since his information was included on the CD in error.

Outcome:

HIMS Chief investigated this incident and found that the CD burning software they use sometimes stores information from the last CD burned. HIMS Chief conducted education and training to ROI staff to ensure this type of issue does not happen again. HIMS Chief counseled the employee who burned the CD. CD is in possession of the Privacy Officer.
