Veteran A requested her medical records. Within the records she received were three EKG reports that had another Veteran's name and full SSN at the top of the page and her name at the bottom of the report. She returned the documents to the release of information department. Update: 04/26/12:One Veteran will be sent a letter offering credit protection services.
