Issue:

Employee A, who is a Veteran, was admitted into the our hospital. The Information Security Officer (ISO) was asked to keep an eye on Employee A's medical record. The ISO found that three other employees accessed the record without authorization or need to know. Employees B and C admitted gaining access without a need to know, Employee D denied gaining access. Update: 08/01/12:Employee/Veteran A will be sent a letter offering credit protection services due to the medical record being accesed without authorization or a need to know.

Outcome:

The incident has been reported to Human Resources (HR) and administrative action is pending.
