Based on record review, the facility failed to protect the private health information (PHI) of Patient A. This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 4/22/13, was reviewed on 4/22/13. The report indicated Staff 1 gave Patient A's laboratory requisition inquiry to Patient B. The requisition was mixed in with Patient B's discharge paper work. The requisition contained the following information: Patient A's name, date of birth, age, sex, room number, medical record number, and account number. The inquiry was returned to the facility.The facility investigated the breach, and provided training and counseling to Staff 1.
