Issue:

Veteran A received an envelope intended for Veteran B at his address. Veteran A stated he did not note the name on the envelope was not his until after he realized the contents of the envelope belonged to Veteran B. The envelope contained eight (8) pages of lab results. One of the lab results was for the Veteran B's HIV test. Update: 07/21/11:Veteran B will receive a letter offering credit protection services due to name and full SSN being disclosed.

Outcome:

The employee who sent the envelope of lab work to the Veteran was identified and the PO met with the employee to discuss the ramifications of this situation. The employee expressed an understanding of the seriousness of the issue and stated that in the future, steps would be taken to ensure this does not occur again. The employee endorsed that future mailings would be validated by another staff member and/or that the clerks would handle future mailings.
