Issue:
On July 23, 2013, the Privacy Officers were made aware of this incident: Veteran A was discharged from an inpatient status on 7/15/13. During a routine follow-up telephone call from VA Nursing Telephone Care, it was learned that Veteran A, upon being discharged, also picked up an information sheet concerning a discharge medication of Veteran B in error. The document Veteran A received only contained the full name, full address and the type of medication of Veteran B. No other personal health information of Veteran B was involved or disclosed. A follow-up telephone call was made to Veteran A by Pharmacy Service. Veteran A confirmed he received an information medication document belonging to Veteran B and that he had already shredded the document.
Outcome:
07/23/13: Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
