Issue:

When Veteran A opened his pharmacy re-fill package from Consolidated Mail order Outpatient Pharmacy (CBOC), he discovered that the medications were for Veteran B. Veteran A informed telephone triage and agreed to return the medications to the nearest CBOC. Veteran A and Veteran B both share the same first name, and have similar sounding (and spelling) last names. The Privacy Officer was informed via a medication error report filed by the telecare Pharmacy technician who indicated that Veteran's B's address may have been inadvertently changed locally, which is where the CMOP derives its address listing. Update: 10/15/12:Veteran B will be sent a notification letter due to protected health information being disclosed.

Outcome:

Employee verbally counseled by supervisor to exercise caution and verify proper patient's record before making changes.
