Issue:

The Birmingham VA Medical Center (BVAMC) Privacy Officer (PO) was notified by an RN that he had faxed a document titled Medicine History and Physical (H&P Note) of the wrong patient to a vendor. The vendor contacted the RN to inform him it was a document of a wrong patient, and the RN asked the vendor to return the document. Vendor agreed to return the document. The information at risk included the patient's name, full SSN and protected health information (PHI) Update: 07/20/12:The patient will receive a letter offering credit protection services.

Outcome:

Document was retrieved and education, training and awareness was given to the employee.
