Issue:

Patient A received two non-controlled substance medications intended for Patient B. This incident was reported to the Loma Linda Healthcare VAMC (station 605) by Patient A and the Loma Linda Healthcare VAMC reported to the Consolidated Mail Outpatient Pharmacy (CMOP). The medications item contained Patient Bs name and medication names. No SSN data was compromised. The Loma Linda Healthcare VAMC will inform the patient to return the medications to the VAMC to be properly disposed of. Investigation reveals that Patient Bs mailing address was inadvertently changed to the same address as Patient A by the Loma Linda Healthcare VAMC as a result of human error. The VAMC has updated the database with the correct address.

Outcome:

01/07/14: Patient B will receive a HIPAA letter of notification.
