Issue:
A progress note was mailed to the wrong patient. When the request was reviewed, it was found that someone had entered Patient B's address on Patient A's request. It is believed (but not confirmed) that an employee was helping Patient A fill out the request and entered the wrong information. The Release of Information (ROI) clerk sent the requested note to Patient B's address. Update: 08/07/12:Patient A will be sent a letter offering credit protection services due to name and full SSN being exposed.
Outcome:
Staff reminded to be more cautious when handling documents with PII.
