Issue:

On 9-16-2011 a request for ROI was processed on two separate patients. The medical record information on both patients was inadvertently placed together and mailed to one of the two requester's. The patient who received his and the information on the other veteran reported this to the facility. The veteran also returned the record copies via hand carrying the medical information back to the Allentown CBOC. The ROI Supervisor will process the information for the other patient and send the information out as requested. Update: 09/26/11:Veteran B will be sent a letter offering credit protection services.

Outcome:

Training was provided to the employee. Double check everything that is being mailed to ensure that the documents are going to the correct addresses. Based on the current staffing in this area there is the need to work at a pace that affords requests to get processed in a timely manner. Despite this we need to take the time to check our work before sending even given if there is a backlog or not.
