Veteran A called after hours on 04/05/11 and left a voicemail regarding having received medical documents on Veteran B. The Privacy Officer (PO) called Veteran A back on 04/06/11 but he was sleeping and wasn't able to get much information from a third party. Veteran A received a document that had Veteran B's name and last four digits of the SSN. The PO will call Veteran A back to get further details. Update: 04/07/11:Veteran B will be sent a notification letter.04/11/11:Monday, April 11, 2011 1:16:00 PM vhawwwbeauclupdate - Veteran A brought in documents that he received of Veteran B. This was an entire 10-10 with complete name, SSN, Address, DOB, phone, next of kin, financial information and military service dates.

Outcome:

PO have met with the supervisor of the department and have requested that they use windowed envelopes for all mailing
