Issue:
A Cardiology Consult Document with full SSN, Full Name, Full Address, Telephone Number, DOB, and diagnosis/appointment reason for two Veterans was found on the sidewalk between the VAMC and Albany Med parking garage on 08/21/13 at approximately 3:00 PM. Two pages were printed, the third page was hand written with the last four and diagnosis of a third Veteran. The documents were secured and brought to IT, then the ISOs were called because the PO was out. An NSOC ticket is being submitted, there is no way to determine how the long the document was in the parking lot or whom may have had access to read.
Outcome:
08/22/13: Two Veterans will receive a letter offering credit protection services. The third Veteran will receive a HIPAA letter of notification.
