Issue:

It was reported by the VA police that an employee in Martinez reported that her locked file cabinet was broken into sometime between when she left work on 06/30/14 and returned in the morning on 07/01/14. The file cabinet contains medical/vision records on approximately 60 patients. The employee is unable to determine if any of the files were compromised. The files have been locked in a secure location pending completion of investigation. VA police are investigating the incident and the police report is being completed.

Outcome:

07/02/14: The incident was reported to OIG. 07/03/14: The Privacy Officer has completed the inventory of the files that were stored inside of the filing cabinet and was able to determine that the amount of files containing information is 80. The files were in a secure cabinet and it was determined that the lock for the filing cabinet was compromised and engineering has placed a more secure lock on the cabinet. VA Police stated that they have interviewed a possible suspect but that the suspect denied involvement. VA Police are continuing with the investigation and will provide updates as they obtain them. All 80 files have been secured by the individual in possession of them. 07/03/14: Due to the fact that it is not known if any of the information was retained by the individual(s) who broke into the file cabinet, the Incident Resolution Service Team has determined that all 80 Veterans will be sent letters offering credit protection services. 07/09/14: It was determined that 15 of the Veterans are deceased, so in those cases, notification letters will be sent to the next of kin.
