Issue:

Based on interview and hospital document review, the hospital failed to prevent the disclosure of Patient Q's PHI (protected health information) to an unauthorized individual.Findings:Review of hospital documentation showed PHI belonging to Patient Q was inappropriately accessed by a staff member on 10/9/2013. On 10/11/13, the hospital discovered a nurse, who was not a part of the care team, accessed Patient Q's medical record without a need to know.Patient Q's PHI was disclosed via "metadata," a virtual chart, and included all of the patient's information (demographic, medical and financial).On 8/29/14 at 1400 hours, a conference call with the Interim Director of the Health Information Management Department confirmed this intentional breach of PHI occurred as documented.
