Issue:

Veteran patient was provided a Fecal Occult Blood Test Kit to take home. Prior to taking the test, Veteran observed that the label on the test was incorrect and informed our Dearborn Community Based Outpatient Clinic (CBOC). The information on the label had a different Veteran name and SSN. Update: 07/16/12:One Veteran will be sent a letter offering credit protection services.

Outcome:

Provided credit monitoring to Veteran. Double check process established to ensure proper mailout in the lab.
