Copied Anesthesia records are missing. Information was sent from one office to another using a pneumatic tube system at the Bay Pines VA Healthcare System. It was reported that the information was never received. Each record contains patient full name, full social, surgery performed, and several pages of anesthesia specific data related to surgery/procedure of 12 Veterans.
Outcome:
08/19/13: Twelve (12) Veterans will be sent letters offering credit protection services.
