Issue:

A VA employee accessed their eOPF files on Friday, July 6th and noticed that the Security Background check coversheet containing the Name and SSN of another employee were commingled with their personnel information. The employee immediately contacted their supervisor, who in turn contacted Human Resources (HR). HR removed the document from the employee's file immediately. The VA contractor in Chantilly, Virginia is responsible for scanning documents into employees files per the HR office at VA Puget Sound. The VA employee whose information was disclosed to the complainant is known and has been identified, but not contacted as of yet, pending guidance from RMIR. Update: 07/09/12:Employee B will be sent a letter offering credit protection services.

Outcome:

Contacted VA contractor ANACOMP in Chantilly, VA of the scanning error in eOPF. ANACOMP scans 500,000 documents per day for the VA.
