Quality Management sent the Privacy Officer (PO) an incident report that was among other things needing to be scanned in to records. The report states that on 08/19/11 Veteran A, received an "Out of Stock" medical communication intended for Veteran B. The report states that the "Out of Stock" medical communication was mailed from Providence. The two Veterans have a similar last name. The information disclosed included Veteran B's partial SSN, last name, first Initial of first name, address, and medical information. Update: 09/02/11:Veteran B will receive a letter of notification.
Outcome:
Sent notification letter to Veteran and educated Pharmacy Staff on importance of double checking to ensure correct address and Patient prior to shipping.
