The Research Compliance Officer (RCO) reports that during a routine audit of HIPAA authorizations for a research study she discovered that five authorizations were missing. After a search, the Principal Investigator (PI) has not been able to find the authorizations.
Outcome:
05/09/14: The five Veterans will be sent HIPAA notifications, as their PHI was potentially disclosed.
