Issue:
A Community Based Outpatient Clinic received a hand written note from Veteran A stating he received his lab results and Veteran B's lab results. He returned the mis-mailed lab letter with his note to the Community Based Outpatient Clinic (CBOC). Update: 11/26/12:Veteran B will be sent a notification letter.NOTE: There were a total of 26 Mis-Handling incidents this reporting period. Because of repetition, the other 25 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Requested Nurse Manager to add the importance of quality assurance when mailing letters to Veterans, ensuring that we are not violating Veteran's privacy by exposing information to others who do not have a need to know. Privacy Officer offered to present privacy training at CBOC staff meeting.
