Issue:

A Veteran with the same first name, middle initial, and last name received medication for another Veteran. Veteran who received the medication via UPS contacted the Indianapolis call center and spoke to a pharmacist there. That pharmacist called the supervisor in pharmacy at Northern Indiana. The medication label had the Veteran's full name, provider name, medication name, number and dosage. Also in the envelope was the drug information sheet with the description of uses, side effects, precautions and interactions of the medication. The diagnosis for the reason the medication being used was also on the sheet. A UPS return package is being sent to the Veteran to have the medication returned to the pharmacy. The Veteran did not take any of the medication. The medication is being sent to the correct Veteran. Update: 12/13/12:One Veteran will be sent a letter of notification.

Outcome:

Pharmacy staff will be re-educated on the importance of double checking and verifying all Veteran information before mailing medications.
