Issue:

The Privacy Officer (PO) received call from Veteran A. He had requested information from a social worker and in the information he found information from Veteran B it appears to be a Patient inquiry: including, name, address, SSN, phone number, NOK, some emergency contact information and insurance information. Also there was also some information about an advance directive on Veteran B. Veteran A has said he will bring the information back to me tomorrow. The PO requested that Veteran A mail in Veteran B information but he did not want to. The PO will update ticket once the information is returned. Update: 04/13/11:Veteran B will be sent a letter offering credit protection services.

Outcome:

PO has worked with Social Work Service regarding this incident. Appropriate action taken regarding staff involved. Training and emphasizing the importance of checking information before mailing out.
