Issue:
On August 8, 2014, an employee, Fee Service, was notified by an individual he inadvertently received a fax on three Veterans which was intended for another recipient. The individual stated the faxes were received as electronic faxes via email but assured the employee, Fee Service, the emails had been deleted. Veterans personal identifiable information includes: Full name, full social security number, medical records, and fee authorization.
Outcome:
08/11/14: The Incident Resolution Service Team has determined that three Veterans will be sent letters offering credit protection services.
