Issue:
Fee Service personnel mailed three claims with medical record documentation attached to another VA Medical Center. The mailing label had the wrong ZIP Code. The envelope was not sent via trackable means. The envelope was returned. There was evidence that the envelope had been opened and re-sealed at least twice. The envelope was tattered and information inside could easily be accessed. Update: 06/30/11:Three Veterans will be sent letters offering credit protection services, as their full SSN's were disclosed.
Outcome:
When mailing PII and other VA sensitive data, Fee staff will send via trackable method.
